If you have signed up for a class please print out all the above forms. If you have answered YES to ANY of the questions on the medical form you MUST see your physician and get clearance for diving, in writing before you can dive with us.
Take your time and thoroughly read each of the waivers. You are waiving certain legal rights, such as the right to sue Jim Wyatt and the Cave Country Dive Shop. Your signature on these forms tells us that you understand the hazards of cave diving and that you voluntarily waive the right to sue us should you be involved in an accident.
